Client Financial Responsibility & Cancellation Policy

At Breakthrough Wellness Centre, we value your time and are committed to providing timely, quality mental health care. Appointment times are reserved specifically for each client. To ensure fairness and availability for all individuals seeking services, the following policy applies to therapy and medication management appointments.

Insurance Participation & Financial Responsibility

Breakthrough Wellness Centre accepts all major commercial PPO insurance carriers.

Insurance coverage varies by individual plan. Clients are responsible for verifying benefits, including:

• Deductibles
• Copayments
• Coinsurance amounts
• Authorization requirements
• Covered services
• Out-of-network benefits (if applicable)

Clients understand they are financially responsible for any balances not covered by insurance, including denied claims, deductible amounts, copays, coinsurance, or non-covered services.

Cancellation and no-show fees are not billable to insurance and remain the sole responsibility of the client.

Cancellation & Rescheduling Policy

Clients must provide 24 hours or more notice during business days to cancel or reschedule an appointment without penalty.

Please note:

• The 24-hour requirement refers to business days only
• Weekends and holidays do not count toward notice requirements
• Messages received after business hours are considered received the following business day

Late Cancellation & Missed Appointment Fees

Appointments cancelled with less than 24 business hours’ notice, or missed without notice, will result in a cancellation fee equal to the reserved service cost:

Therapy Appointment:

$200 cancellation fee

Medication Management Appointment:

$250 cancellation fee

These fees are due directly to Breakthrough Wellness Centre and cannot be submitted to insurance carriers for reimbursement.

Same-Week Rescheduling Consideration

At the discretion of the treating clinician, clients may be offered an alternate appointment within the same calendar week, if availability permits.

Rescheduling accommodations are not guaranteed and depend entirely upon provider availability.

Emergency Exceptions

Breakthrough Wellness Centre understands emergencies, sudden illness, and unforeseen circumstances may occur. Exceptions may be considered on a case-by-case basis at provider discretion.
